Do you Bill health insurance?

We do not bill health insurance, only P.I.P and LNI (work injuries). We do offer to create a superbill for you to submit to your insurance for reimbursement. It is, however, up to you to know your benefits and what your insurance plan will allow.

We need a referral from your provider that includes your name, DOB, Diagnosis, Date Of Injury and the referring providers NPI number. We will also need your claim information which should include the insurance name, claim number and how to contact your adjustor.

What is needed to be seen under LNI or a P.I.P claim?

what is a superbill?

A superbill is a detailed invoice outlining the services you received. As the client, you can submit the superbill directly to your insurer, giving the insurer all the information they need to reimburse for your session OR it may go to your deductible. There will be space for you insurance information, diagnosis, treatment codes etc. as long as they are provided to us by you or your doctor; otherwise, you will need to fill it out upon receiving. All superbills will be e-mailed to you by Meghan unless you request otherwise.

why do i have to fill out an intake form?

Health intakes are required by law before any health service can happen, including massage. We are required by HIPAA to keep all of your information private. We only use the information on your intake form to make an informed decision about your treatment on the table. Your session can not proceed with out these forms filled out.

We ask for a 24 hour notice of cancellation. If that is not provided, you are at risk of being charged out of respect for our time and energy. Illness and emergencies will be assessed on a case by case basis.

What is your cancellation policy?

Our policy on minors is that no one under the age of 12 can receive a massage here, we are happy to help you find someone that can. Minors 12 and older all need a waiver signed by a parent or guardian prior to their session and kids age 12-15 will need a parent or guardian present either in the room or within earshot during the entirety of the massage.

Can my minor child get a massage?
